Our mission is to provide participants with opportunities to make person-centered choices and to empower them in making appropriate decisions that reflect principles of self-determination.
Individual Service Plans (ISP) and Individual Plans (IP) are developed with input by the participant and circle of friends that include members of the family, assigned case manager, guardian, and significant others identified by the participant.
The plan describes what is important and
meaningful to the participant to enhance the
individual's life experiences and quality of life.
Self-determination and person-centered
planning are reflected not only in the Adult Day
Health program but also in other services identified
by the participant in the Individual Service Plan
and Individual Plan.
Person-centered planning empowers participants to make appropriate choices towards inclusion in the community.
Once enrolled in the program, the participant's Individual Plan is developed which outlines the individual's choices for implementation.
